What options do I have
for starting a family?

It may be difficult to get pregnant when 
you have primary ovarian insufficiency (POI), 
but it is not impossible

Some people with POI might be able to have a baby without help, but this doesn't happen very often.

About 5-10% of people with POI or 5-10 out of 100 get pregnant on their own. Most people need some kind of medical help to get pregnant. Adoption can also be an option for people with POI.

If you have POI and do not want to get pregnant, use birth control such as pills, condoms, or IUDs. Some POI treatments also work as birth control.

People with POI may still ovulate because POI is not a simple switch from "on" to "off"

This does not happen to everyone with POI. Healthcare providers can use monthly ultrasounds of the ovaries to check if patients with POI are ovulating. This helps determine the best time for the patient to try to get pregnant based on signs of ovulation.

Options may include special approaches to getting pregnant. This may include intrauterine insemination (IUI) or in vitro fertilization (IVF).

In IUI, sperm is injected into the uterus to help someone get pregnant. This sperm can be from a partner or from a donor.

Clomid and Letrozole

Clomid and Letrozole are commonly used to treat infertility. They work by helping the ovaries release an egg. Unfortunately, these medications often do not work for people with POI because their ovaries do not produce estrogen. Freezing eggs for later use doesn't work for people with POI because there aren't enough eggs left to collect and freeze.

The most successful way to become pregnant with POI is by using eggs donated by another person

Donated eggs are fertilized with sperm and then placed into the uterus. This is a version of in-vitro fertilization, or IVF.

Discussing family planning with a healthcare provider is important. If pregnancy is your goal, ask for a referral to a fertility specialist.

Fertility specialists can discuss all available options to determine what makes the most sense for each person’s needs. Not all individuals with POI will decide to have children.
